Description
This Excel spreadsheet provides an intuitive, simple form for entering patient responses in the “Data Entry” worksheet. It calculates all KCCQ scale scores by routines validated against canonical test data and stores each patient record (patient ID, date of study, scale scores and raw responses) as a separate row in the “Data” worksheet.
System Requirements: Windows 95 or better; Excel 97 or better.
