Description
This Excel spreadsheet provides an intuitive, simple form for entering patient responses in the “Data Entry” worksheet. It calculates all KCCQ-12 scale scores by routines validated against canonical test data and stores each patient record (patient ID, date of study, scale scores and raw responses) as a separate row in the “Data” worksheet.
System Requirements: Windows 95 or better; Excel 97 or better.
