Description
This Excel spreadsheet provides an intuitive, simple form for entering patient responses in the “Data Entry” worksheet. It calculates all PAQ scale scores by routines validated against canonical test data and stores each patient record (patient ID, date of study, scale scores and raw responses) as a separate row in the “Data” worksheet.
System requirements: Mac OS X 10.2.8 or better; Excel 10.1.5 or better — but incompatible with Excel 2008 and later at this point.
