About the Kansas City Cardiomyopathy-12 Questionnaire

The 12-item Kansas City Cardiomyopathy Questionnaire (KCCQ-12) is a shortened version of the original 23-item KCCQ. It generates the Symptoms, Physical and Social Limitations, Quality of Life and Summary Scores, but not the Symptom Stability and Self-Efficacy scales. It is highly correlated with the original, as described here: https://www.ahajournals.org/doi/10.1161/CIRCOUTCOMES.115.001958.

       It was designed to decrease the response burden on patients to make it much more feasible for use in routine clinical care. On average, it takes 2-4 minutes to complete. It has been endorsed by the International Consortium on Health Outcomes Measurement to measure the value of heart failure care.
